A 65-year-old man presents with a 2-week history of a rash that started on the back of his neck and spread downward to his back, chest, and arms.

On exam:
- Confluent red, scaly plaques covering ~90% of his body
- A distinct orange-red hue
- Well-demarcated patches of normal skin WITHIN the affected areas
- Waxy, thickened skin on the palms and soles

At a glance it looks like erythrodermic psoriasis. But those normal-skin "islands" and the waxy palms are telling a different story.

The teaching pearl for dermatology NPs: PRP is the great psoriasis mimic. The tells are the orange-red hue, "islands of sparing" (normal skin sitting inside the plaques), and waxy palmoplantar keratoderma. When a "psoriasis" case doesn't quite fit — or isn't responding to psoriasis therapy — PRP belongs on your differential. Biopsy confirms, and systemic retinoids like acitretin are first-line.

The P value is one of the most cited and most misunderstood — numbers you'll see in dermatology research.

If you've ever read a journal article and seen P < 0.05, it's easy to assume it means "this treatment works." But that's not actually what it tells us.

A P value measures the probability of observing results this large (or larger) if there were truly no real difference between the groups being studied. Traditionally, a P value below 0.05 is considered "statistically significant," meaning the findings are less likely to be due to chance alone.

But here's the important part for Dermatology NPs:
Statistical significance is not the same as clinical significance.

Understanding what a P value can and can't tell us, is one step toward becoming a more confident consumer of dermatology research.

A 34-year-old man with 20+ years of severe, treatment-resistant psoriasis (80–90% BSA, erythroderma) has failed nearly everything – topicals, phototherapy, acitretin, five biologics, and apremilast.

He's also got a history of gout. His provider starts him on cyclosporine and lisinopril to finally get the psoriasis under control.

In recognition of National Psoriasis Awareness Month, SDNP Past-President Melodie Young, MSN, A/GNP-C, FSDNP, DCNP reminds us that every patient deserves a provider willing to look beyond the plaques.

Psoriasis is a complex, systemic inflammatory disease that can be associated with psoriatic arthritis, cardiovascular disease, metabolic syndrome, depression, and other significant comorbidities. As dermatology nurse practitioners, we have an opportunity, and a responsibility, to go the extra mile.

Take the time to ask the additional questions. Screen for associated conditions. Educate your patients. Collaborate with other specialties when needed. Those extra few minutes can make a lasting difference in a patient's health and quality of life.
